Key Care Coordinator Responsibilities

  • Creating detailed, person-centred care plans together with care home residents
  • Coordinating with other professionals involved in residents’ care, where necessary
  • Ensuring care plans are kept up-to-date
  • Supporting the care team to effectively execute care plans
  • Provide the care team with necessary training
  • Documenting the wellbeing of residents

Required Skills & Qualifications

  • A degree in nursing, social care or a related field
  • Experience in a care home setting – desirable but not essential
  • Thorough understanding of principals and legislation underpinning elderly care
  • Great communication skills, both written and oral, in order to build positive relationships with care home residents, staff and residents’ family members, and other healthcare professionals
  • Time management to coordinate and organise tasks in your care coordination tasks
  • Ability to work well as part of a team
  • Attention to detail, ensuring care plans are accurate and truly reflect the needs and wishes of the residents
  • A valid driver’s licence (for community-based roles)
